Subject: | Australian Yak-52 warning notice - elevator bellcrank |
As you may recall, there was a fatal accident involving a Yak-52 in Australia in
2019. The accident report is in review, but the Australian Transport Safety
Bureau (ATSB) have released a safety bulletin regarding one of their findings.
The highlighted issue relates to the elevator bellcrank, an issue I'm sure the
members of this forum have heard before.
For reference, the ATSB notice is at:
https://www.atsb.gov.au/publications/safety-advisory-notice/ao-2019-027-san-024/

One unanswered question. What was the condition of the 4 bolts and nuts securing
the elevator counterweight to the bell rank? I guarantee if those bolts were
not properly secured, that counterweight moves and put significant side loads
on the bell tank. That side loading may be the cause of original bell rank cracking.
Those bolts and nuts should be checked at each inspection and marked
with torque seal. How many of them have been other than visually inspected?
