Thanks Carlos -- Yep, that's it. Someone else suggested I
Google it, which I hadn't though of doing, and there it is.

Is this what you are looking for?
http://www.faa.gov/licenses_certificates/aircraft_certification/aircraft_
registry/media/AC8050-1B.pdf
I found it here:

My 601 XL is due for registration renewal soon,
and I received the notification in the mail. It's only a $5 fee for
three years, and looks like a very simple process. It can be done
online, but their site won't work. Otherwise, AC form 8050-1B is
required, so I looked for it on the FAA website. I can only find form
8050-1A, which is perhaps the predecessor to 1B. The phone lines are
forever busy, so e-mailed an FAA official in an attempt to get the
proper form. After a couple exchanges, he sent the form as an attachment
to the e-mail, but the attachment got dropped somewhere.
I guess this is a classic experience with
government bureaucracy. Maybe someone on this list has a form 8050-1B
available to scan and e-mail, or know where I can find one.
