Boy John,
Tough one. I have see some guys bend some angle, using the hardwood form and some
pipe clamps.(do a Google search for pipe clamps). You clamp the starting point
of the bend, and then slowly work your way along the radius bend until you
have your 14". That would be a trial-and-error thing for me.

Does anyone know what kind of shop would have a tool that would put a nice smooth
4-foot radius bend 14" long into a piece of extruded angle 6061-T6, 1.25" x
0.75" x 0.125t, with the bend stretching the outer edge of the 0.75 flange?
This is aluminum angle with the radiused inner corner.
I can probably do it with a grooved hardwood form and a rubber mallet, but I'd
gladly pay somebody to do it for me that could assure perfect results the first
time.

You said the evil words, Keith, "trial and error." I got usable results on
an 18" piece of 3/4 x 3/4 x 0.090 just clamping it in the vise and
manhandling it a little at a time, but this time I want to bend the last 14"
of two 6.5-foot pieces, and that stuff is expensive. I just ordered some
shorter pieces to practice on in case I couldn't find someone who is
equipped to do it right the first time, but I'd rather not do trial
and error.

It really doesn't need to be trial and error. If you control to process and keep
the bending between the tangent lines, you just sneak up on it. The extruded
angles will need to be bent and flattened as you go along.
